Ohio Transfer to Minors Account (OTMA)
Funds deposited into this interest bearing account represent an irrevocable gift to the minor.
The funds may be used only for the benefit of the minor paid over to the minor by the custodian
for use by the minor. This account is classified as a Statement Savings Account that has the
monthly maintenance fee waived while the child is still a minor. Once the child turns eighteen
(18) years of age this account will be changed to a Statement Savings Account. (Please see

Your interest is computed daily and paid monthly. You will receive a monthly statement showing
all your transactions made during the statement period along with your Annual Percentage Yield
(APY) earned.
